The day before embryo transfer, the progesterone, P4, level is generally expected to be above 10 ng/mL. If it is below 10, the implantation rate and the chance of ongoing pregnancy may decrease significantly, so supplementation is needed.

Once implantation occurs, we also hope to keep P4 between 10 and 30 to help maintain a more stable pregnancy.

Progesterone supplementation usually includes oral medication, vaginal suppositories, and injectable oil-based or water-based progesterone.

  1. Oil-based progesterone injection:
    It is not convenient for self-injection, tends to be more painful, may cause redness and swelling, and some people may have allergic reactions. However, its effectiveness is good.
  2. Water-based progesterone injection:
    It can be self-injected and is less painful. However, the blood concentration is lower than that of oil-based injections, the number of injection days is higher, and the cost is more expensive.

Each option has its advantages, but for those who feel faint at the sight of needles, injections can still be intimidating.

We had a patient preparing for embryo transfer who came from Hong Kong. Her progesterone level was not sufficient. After much explanation and encouragement from Dr. Li, she finally agreed to receive the oil-based injection.

After the injection, we asked her if she felt any discomfort. She said it was actually fine 😅 and acceptable.
