Today in clinic, I saw three women in a row.
All of them were over 40 years old, with AMH levels between 0.5 and 1.1.
When they previously received treatment at other fertility centers, no more than three eggs were retrieved.
This is what we call a poor responder.
For both patients and physicians, poor response to medication is a major challenge.
Various stimulation protocols and adjunctive treatments have been proposed, all with the goal of improving the pregnancy success rate for poor responders 🤱🏻.
Dehydroepiandrosterone, DHEA, is one adjunctive medication that is often mentioned.
Its role is to help increase the number of antral follicles, allowing multiple follicles to grow together during ovarian stimulation, which may lead to the retrieval of more eggs.
In most clinical studies, DHEA is started three months before IVF treatment, at 75 mg per day, and continued until the end of ovarian stimulation, with the goal of increasing the number of eggs retrieved.
